Let me know why you are interested in my practice and your
insurance company (if any). I will place you on the
waitlist.
2.
If accepted, you will receive a confirmation email from me with an
invitation to begin the new patient registration procedure (this
list).
3. Print out the New Patient Packet and fill it out completely! Click here.
Please bring it with you to the appointment. This packet has the
practice's policies, a checklist for what to bring to your appointment,
and a signature page that allows me to treat you. Please give the Authorization to Disclose Information form to your previous physician prior to your appointment in order to transfer your old records to me.
4. Print out the Medical History Questionnaire. Please download the history form, print it, and fill it out by clicking here. Be aware, it should take approximately 1 hour to complete and there is a 3 day diet diary.
All patients should come to the new patient visit with all active
prescription medications in hand, and be prepared to discuss their past
medical history in detail. Preparing beforehand by completing the
survey will help patients get more out of their office visit.
5. Decide whether you would like me to be your Primary Care Physician or a Consultant. If
you'd like me to be your Primary Care Physician, call your
insurance company by calling the customer service number written on
your insurance card and let them know. However, if you would
like me to do a Holistic Medicine Consultation, just ask your
doctor for a verbal or written referral to see me. That way, you
can see me as well as your primary care doctor!
6. Transfer your medical records. Each
new patient should request a transfer of medical records from their
previous physician using the form that is available in the New Patient
Packet (see above- it's called the Authorization to Disclose
Information form). Inquire of the cost of transferring records
before submitting the form! If costs are prohibitive, you should
contact Dr. Hirsch to decide which elements of the medical record are
essential. Patients can be seen before I receive this information, but
please request the transfer before your appointment.
6. Schedule a "New Patient Visit" by clicking here.
Your visit will last 2 hours. Children under 18 with non-eventful
past medical histories may schedule a "Well Child Office Visit" (60
minutes) for the new patient visit. Previous St. Peter Family Medicine patients: Please schedule a "New Patient" visit as well.
Be
aware, for most patients with complex or chronic illness it will take a
few visits before our plan for healing is solidified.
