Let us know why you are interested in our practice and your
insurance company (if any). We will place you on the
waiting list.
2. When accepted, you will receive a confirmation email from us with an
invitation to begin the new patient registration procedure (this
list).
3. Schedule a New Patient Visit.
Your visit will last 1 hour. Children under 18 with non-eventful
past medical histories may schedule a "Well Child Visit" (60
minutes) for their first visit. Click below for the appropriate history form. Please
write down your login information and password for the online
appointment scheduler, and change it to something you will remember
(recommended). 4.
Print out the New Patient Packet - review it and bring the Patient
Information page, the Signature Page, your annual fee and any questions
that you have on the Practice Policies with you to your appointment. Please bring it with you to the appointment. This packet has the
practice's policies, a checklist for what to bring to your appointment,
and a signature page that allows me to treat you.
5. Please print out the Adult Medical History Questionnaire or Well Child Exam Form as appropriate for age - fill it out and bring it with you to your first appointment. 6. Decide whether you would like us to be your Primary Care Physician or a Consultant. If
you would like Dr. Hirsch or Margie Miller to be your Primary Care Physician, call your
insurance company by calling the customer service number written on
your insurance card and let them know. However, if you would
like to do a Holistic Medicine Consultation, just ask your
doctor for a verbal or written referral to see us. That way, you
can see us as well as your primary care doctor!
*6. Transfer your medical records. Each
new patient should request a transfer of medical records from their
previous physician using the form that is available in the New Patient
Packet (it's called the Authorization to Disclose
Information form). Inquire of the cost of transferring records
before submitting the form! If costs are prohibitive, you should
contact us to decide which elements of the medical record are
essential. Patients can be seen before we receive this information, but
please request the transfer before your appointment.
Be
aware, for most patients with complex or chronic illness it will take a
few visits before our plan for healing is solidified.
